At the 
May 21 Senior Awards Ceremony, the PTSA will award TWO $500 Scholarships to two outstanding seniors.

Applications must be turned into Student Services by MAY 1.
DETAILS:
- Applicant must be a senior at Athens Drive High School, graduating in June 2015.
- Applicants must be a member of the Athens Drive PTSA as of May 7, 2015: membership form attached OR go to https://athensdriveptsa.com/join-athens-drive-high-school-ptsa/ OR email PTSA Membership Committee Chair, Ms. Luisa Martin-Price at martin@gmail.com to check your membership status.
- Complete the attached “PTSA SCHOLARSHIP AWARD APPLICATION & INSTRUCTIONS” form.
- Complete responses to the attached “ADDITIONAL INFORMATION,” typing responses on a separate sheet(s) of paper.
- Attach one letter of recommendation from an administrator, counselor, teacher, or person in charge of any service organization in which you are active.
- Attach an official Athens Drive High School transcript, which can be obtained from Student Services.
- If applicant is under the age of 18, a parent/legal guardian must sign the Parent/Guardian Authorization on page one to allow the PTSA Scholarship committee to obtain a transcript from Student Services.
Application must be submitted to Student Services no later than FRIDAY, MAY 1, 2015
Winners of the annual PTSA scholarship will be announced at The Senior Awards on May 21, 2015.
** Scholarship funds will be sent directly to the school of your choice. Additional information will be obtained from the scholarship recipients prior to the disbursement of funds.
